Patient Forms

Upon entering our office, Hilda, our Office Administrator, will greet you and welcome you as a member of our Chiropractic practice "family".  We will request that you fill out our patient forms located in the "New Patient Center". This paperwork provides us with general information about yourself and your condition(s).

Consultation

Next you will have a consultation with our Chiropractors, Drs. Pisarek, Gurvey or Dr. Brooks, to discuss your health-related problems, concerns, as well as potential treatment options. Your first visit is designed for the 'Health Team" professionals to learn more about you, your condition(s) and your expectations, to determine whether Chiropractic or massage care at our facility will meet your goals.

Examination

After your Chiropractic consultation, your Chiropractor will perform a complete Chiropractic examination testing your reflexes, your ability to turn and bend, muscle strength, as well as other standard postural, orthopaedic, neurologic and physical examinations. Nothing will be done in our facility without your consent. Also, we include "MyoVison" testing utilizing a computerized spinal examination of muscle tension and nerve stress levels.

X-Ray Studies

Your specific condition may require us to take x-rays in our premises or at a nearby radiology centre, to get a full spinal/joint evaluation of you.  X-rays help us rule out more serious conditions or assist us in developing the most effective treatment plan for you.  Most people are amazed once they view their x-rays and can often immediately identify their misalignments and degeneration themselves.

Report of Findings

Once all the information and examinations have been performed, your Chiropractor will arrange for your report of findings visit to answer the four most popular questions:

• Can you help me?
• What do you recommend that I do to get better?
• How long is it going to take?, and,
• How much is it going to cost?

After discussing with you your history and your goals, examining your spine and reviewing any x-rays or tests, Drs. Pisarek, Gurvey or Brooks will discuss recommendations and "informed consent" with you. If we believe that Chiropractic care can help you, we will then recommend treatment and a wellness program.  If you have a condition that requires care from other health providers, then the doctor will tell you and recommend a referral elsewhere. Keep in mind that we only accept patients for care that we honestly feel will benefit from the health services that we provide at our facility.
